Over the last year, we've built the leading virtual cardiac recovery and management platform for members with cardiac conditions. For every member we serve, we add an average of five years to their lifespan.

Compensation: $25/hour (Part-Time, 20-40 hours/week, Pacific Time hours)

The Cardiac Recovery Program is a comprehensive, virtual cardiac rehabilitation initiative designed to help patients recover from cardiac events, adopt sustainable lifestyle modifications, and navigate the complexities of the healthcare system from the comfort of home. Our program combines evidence-based exercise, health education, and patient advocacy to drive meaningful outcomes for those with cardiovascular disease

About the Role

As a Cardiac Recovery Patient Navigator & Exercise Physiologist, you will support patients through every phase of their cardiac recovery journey. You will blend the skills of a patient navigator-helping patients access care, overcome barriers, and understand the healthcare system-with the clinical expertise of an exercise physiologist, guiding safe exercise and lifestyle modification. Your work will be central to helping patients build confidence, improve their health, and sustain long-term recovery

Key Responsibilities

  • Patient Navigation & Advocacy



  • Conduct person-centered interviews to understand patients' life stories, strengths, needs, and recovery goals.
  • Assist patients in identifying and accessing appropriate healthcare providers, services, and community resources.
  • Coordinate care transitions between healthcare settings to ensure continuity of care.
  • Facilitate communication between patients, caregivers, and healthcare providers, ensuring patient needs and preferences are addressed.
  • Support patients in overcoming barriers such as transportation, financial constraints, or cultural differences.
  • Educate patients on navigating health insurance, referrals, and care management processes.
  • Empower patients to actively participate in their healthcare decisions and treatment planning
  • Exercise Physiology & Lifestyle Modification



  • Review patient health information prior to setting individualized recovery goals.
  • Lead patients-primarily older adults with cardiovascular conditions-through safe, evidence-based exercise programs via telehealth/video conferencing.
  • Implement and update exercise plans according to current professional and organizational standards.
  • Monitor patient progress, evaluate compliance, and adjust exercise regimens as needed.
  • Lead group exercise sessions and provide one-on-one coaching.
  • Educate patients on heart-healthy lifestyle changes, including nutrition, stress management, medication adherence, and risk factor reduction
  • Document all interactions and progress in an accurate, concise, and timely manner.
  • Collaboration & Quality Improvement



  • Work closely with health coaches, nurses, physicians, technologists, and the broader clinical operations team to optimize patient outcomes.
  • Participate in ongoing quality improvement initiatives to enhance program effectiveness and patient satisfaction

Qualifications

  • Bachelor's degree in Exercise Physiology, Health Sciences, Kinesiology, Social Work, or a related field.
  • ACSM or AACVPR Certification required (e.g., Certified Cardiac Rehabilitation Professional, Certified Exercise Physiologist, Clinical Exercise Physiologist, or Registered Clinical Exercise Physiologist).
  • Minimum 2 years of experience in exercise physiology, cardiac rehabilitation, patient navigation, or case management, with a focus on cardiovascular health.
  • Strong understanding of the healthcare system, community resources, and cardiac rehabilitation best practices.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Experience working with diverse populations and addressing unique patient needs.
  • Tech-savvy, dependable, and detail-oriented.
  • Bilingual (English/Spanish) is a plus, but not required.
  • Note: This is a 1099 contractor position only.
