Role Description

Zócalo Health is seeking a Community Health Worker (CHW) to support the DHCS Community Health Worker Benefit under Medi-Cal. This role will work exclusively with Medi-Cal members enrolled in the CHW benefit across multiple Managed Care Organizations (MCOs) and counties throughout California. All care is delivered virtually through telehealth. The CHW will play a critical role in building trusting relationships, addressing social needs, and coordinating services for patients—especially those from underserved and geographically diverse communities.

This position reports to the Community Health Worker Manager.

Key Responsibilities

  • Serve as a primary point of contact for patients enrolled in the CHW benefit, building relationships through virtual channels and local staff collaboration.
  • Conduct comprehensive assessments of patient needs related to social drivers of health, including housing, food, transportation, employment, and more.
  • Coordinate and connect patients to appropriate community resources, benefits, and programs across various California counties.
  • Collaborate with local team members to enhance resource navigation and community integration.
  • Support patients in goal setting and follow-up, ensuring consistent engagement and measurable progress.
  • Document all patient interactions and outreach activities in the EHR in compliance with DHCS, health plan, and internal documentation standards.
  • Participate in clinical team huddles, care conferences, and interdisciplinary meetings to ensure alignment with medical, behavioral, and social care plans.
  • Maintain current knowledge of statewide and regional community resources and update internal tools to reflect available services.
  • Assist with patient education, preventive health reminders, and closing care gaps as needed.

Qualifications

Language/Culture

  • Fluency (verbal and written) in English and Spanish.
  • Knowledgeable of Latino customs and cultural norms (preferred)

Education

  • High school diploma or GED.

Licenses/Certifications

  • Must possess a Community Health Worker certification, which included field experience as a requirement for completion -OR-
  • Demonstration of 2,000+ hours of CHW work (paid or volunteer) in the past three years and willingness to obtain a Community Health Worker certification within 18 months of hire date

Experience

  • At least 2 years of experience working as a CHW, care navigator, case manager, or in a related patient-facing role.
  • Lived experience in and deep understanding of Latino communities and Medi-Cal populations.
  • Experience working within or navigating Medi-Cal systems and benefits highly preferred.
  • Comfortable working in a virtual care setting, using telehealth platforms, EHRs, and digital communication tools.
  • Proven ability to manage time effectively, maintain accurate records, and support a high volume of patients remotely.
  • Passion for health equity, cultural humility, and building trust with vulnerable populations.

Complementary competencies and skills

  • Comfortable working with multiple computer applications simultaneously and willingness to learn new technologies and frameworks.
  • Team player who builds effective working relationships.
  • Ability to train others.
  • Well-known in and have strong ties to the local Latino community. (preferred)
  • Well versed in local resources to support SDOH needs. (preferred)
