Job Title: Disease Management Registered Nurse

Pay: $39 - 41/hr (Based on experience, location, license, etc.)

Location: Remote

Job Type: Temp. to hire

Work Schedule: Day shift (40 hours/weekly) - Mon to Fri

Job Summary

Seeking an experienced Registered Nurse (RN) to provide Disease Management (DM) services. This role involves assessing, planning, implementing, coordinating, monitoring, and evaluating patients' health and psychosocial needs to empower them in managing chronic conditions. The DM Nurse will collaborate with an interdisciplinary care team to identify appropriate interventions, prevent complications, and minimize the risk of exacerbation using evidence-based, condition-specific education and coaching.

This position requires the ability to develop individualized care plans in partnership with providers, patients, and families, while continuously assessing the effectiveness of interventions to improve health outcomes.

Essential Requirements

  • Active, unrestricted RN license to practice independently within the state of residence.
  • Must be able to pass a Department of Defense (DoD) background investigation.
  • Ability to obtain multi-state licensure if residing in a Nurse Licensure Compact (NLC) state.
  • Minimum of three years of clinical experience in a healthcare setting.
  • Experience in case management, care coordination, or discharge planning in an acute, ambulatory, or specialty disease management program within a healthcare system or managed care organization (MCO).
  • Previous Disease Management experience within a managed care organization (MCO).
  • Bachelor’s degree in nursing or a related health field is preferred, but not required
  • Knowledge of government healthcare programs, including TRICARE, Medicare, and Medicaid is highly preferred
