Key Responsibilities

  • Serve as a vital liaison between patients, healthcare providers, and insurance partners, facilitating seamless and compassionate care.
  • Assess patients’ medical requirements to develop comprehensive care plans post-discharge, coordinating with hospitals, home health services, and pharmacies.
  • Engage patients and their families in discussions to ensure understanding of treatment plans and follow-up steps.
  • Review health insurance benefit plans to align coverage options with patient needs and advocate for access to necessary resources.
  • Cultivate strong relationships with healthcare providers and specialists, ensuring effective communication and coordinated care.
  • Maintain thorough documentation of patient interactions and care plans, participating in team meetings to address patient needs and barriers.

Qualifications

  • Bachelor's degree in Nursing (BSN) or Social Work (BSW) with an active and clean MA license.
