Key Responsibilities
- Serve as a vital liaison between patients, healthcare providers, and insurance partners, facilitating seamless and compassionate care.
- Assess patients’ medical requirements to develop comprehensive care plans post-discharge, coordinating with hospitals, home health services, and pharmacies.
- Engage patients and their families in discussions to ensure understanding of treatment plans and follow-up steps.
- Review health insurance benefit plans to align coverage options with patient needs and advocate for access to necessary resources.
- Cultivate strong relationships with healthcare providers and specialists, ensuring effective communication and coordinated care.
- Maintain thorough documentation of patient interactions and care plans, participating in team meetings to address patient needs and barriers.
- A compassionate and detail-oriented approach to patient care.
- Strong problem-solving abilities with a proactive mindset to navigate challenges.
- Proficiency in remote work technologies and virtual communication tools.
- Bachelor's degree in Nursing (BSN) or Social Work (BSW) with an active and clean MA license.
- Minimum of 3 years of experience in case management, managed care, or care coordination.
