We are in search of a committed individual to join our Veterans Directed Home and Community Based Services Program (VD-HCBS) as a Case Manager on a part-time, remote basis. This role entails close collaboration with the Program Manager to optimize both fiscal and programmatic functions, ensuring the seamless delivery of services to the veterans we support.
Key Responsibilities
- Conduct engaging training sessions for veterans and their representatives.
- Perform quarterly home visits for assigned veterans to assess and fulfill their needs.
- Manage the intake process for new VD-HCBS referrals, ensuring all enrollment paperwork is submitted to the appropriate financial management services and the Department of Veterans Affairs.
- Maintain comprehensive veteran files and meticulous documentation of all correspondence related to VD-HCBS referrals.
- Review and verify veterans’ timesheets, ensuring accuracy in the electronic database.
- Develop and monitor veterans' budget plans for precise financial reporting.
- Assess veterans’ needs to ensure the appropriateness and effectiveness of provided services.
- Collaborate with the Program Manager regarding new enrollments, changes in veteran status, and any issue resolution.
- Communicate any veteran-related issues with the Program Manager and assist with necessary paperwork approvals.
- Liaise with financial management services concerning enrollments and budget adjustments.
- Produce ad hoc reports as required and maintain the VD-HCBS Monthly Update Spreadsheet.
- Conduct background checks for employees selected by veterans for hire.
- Create and update summary pages for each veteran as needed.
- Submit detailed reports of home visit or phone call activities and complete mileage reports in designated software.
- Engage in meetings and conference calls with local and national VA representatives.
- Adhere to all security and confidentiality protocols.
- Proven ability to prioritize and execute tasks while adjusting to evolving needs.
- Exceptional customer service skills, demonstrating courtesy and professionalism.
- Team-oriented with a collaborative approach.
- Strong listening and information-gathering capabilities.
- Ability to work independently while managing multiple projects.
- Familiarity with federal and state-funded programs.
- Proficient in Microsoft Word and Excel.
- Excellent written and oral communication skills.
- Consistent ability to meet deadlines.
- Valid driver’s license and willingness to travel to various locations.
- An Associate’s or Bachelor’s degree is preferable, accompanied by a minimum of 2 years of case management experience. A combination of education and relevant experience will also be taken into account.
