What You’ll Do
- Empower Patients: Use proven strategies to engage, educate, and coach patients and caregivers, helping them navigate health systems and improve outcomes.
- Address Social Drivers of Health: Connect patients with essential resources—housing, food, healthcare access—and ensure their needs are met.
- Collaborate & Communicate: Participate in and support the effective evaluation planning, development, and execution of patient care plan in collaboration with the patient’s care team
- Build Trust: Forge strong relationships using motivational interviewing and culturally competent care.
- Promote Independence: Coach patients in problem-solving, self-care, and advocacy, supporting their journey to better health.
- Leverage Technology: Use digital tools to connect patients with resources and track progress.
- Document & Support: Document patient encounters, referral activities, and other pertinent information in electronic health record ensuring all encounter notes are completed and finalized per company policy.
- High school diploma or GED required; candidates with relevant lived experience are welcome.
- Prior healthcare experience preferred, especially in geriatric care
- Strong problem-solving, organizational, and multitasking abilities
- Effective communicator—both written and verbal
- Comfortable managing complex patients and diverse needs in a fast-paced, team-oriented environment
- Proficient with Microsoft Office Suite and electronic medical records (EMR); skilled in using internet-based tools and documentation
- Demonstrates professionalism and reliability
- Reside in the state of California
