Belong Health is seeking a skilled Vice President of Care Navigation to lead the execution of new partner implementations, reporting to the SVP of Health Plan Operations. This role covers the entire implementation lifecycle, from pre-signature planning to post-signature execution and transition into steady-state operations.
Essential Job Duties and Responsibilities
This is a remote role that requires travel for key client engagements, team offsites, and strategic planning sessions.
Salary
$170,000 - $220,000 Annually

Belong Health is seeking a skilled Vice President of Care Navigation to lead the execution of new partner implementations, reporting to the SVP of Health Plan Operations. This role covers the entire implementation lifecycle, from pre-signature planning to post-signature execution and transition into steady-state operations.
Essential Job Duties and Responsibilities
- Inspire and manage high-performing clinical and non-clinical teams, fostering a culture of excellence and continuous improvement. Mentor talent across care navigation, nursing, and clinical operations to align with organizational goals.
- Collaborate with partners to enhance the Model of Care. Design and refine patient-centric clinical programs and establish evidence-based workflows and best practices to improve patient outcomes and provider efficiency.
- Direct scalable care navigation strategies in a fast-paced environment. Implement KPIs to measure clinical success, patient experience, and operational efficiency. Monitor data to inform strategic priorities and enhance processes. Drive the adoption of AI and automation tools to improve clinical decision support and care coordination.
- Ensure compliance with internal policies and all applicable regulations. Lead quality assurance initiatives to promote continuous learning and improvement. Act as a clinical liaison for onboarding and integrating new health plan partnerships.
- Partner with internal departments and external stakeholders to deliver innovative solutions. Maintain transparent communication with stakeholders through regular reporting of performance metrics and strategic insights. Represent Belong Health in client meetings and industry forums.
- 10+ years of progressive leadership in clinical operations or related healthcare domains, with a proven track record in mid-to-senior management roles. Experience in early-stage companies is preferred.
- Deep understanding of Care Coordination requirements and compliance across Medicaid, Medicare, and Medicare Advantage programs. Clinical proficiency in managing mental health and chronic conditions.
- Demonstrated ability in designing and scaling patient-centric clinical programs. Skilled in process improvement and problem-solving in dynamic healthcare settings.
- Competence in integrating health risk assessment systems for individualized care planning.
- Experience with technology-enabled care delivery models and digital engagement tools. Knowledge of AI, automation, and analytics in healthcare, with the ability to translate technical capabilities into operational impact.
- Proven success in leading Care Management operations within Medicaid, Medicare Advantage, or ACO environments, including managing readiness reviews and audits with regulatory agencies.
- Master's degree in nursing, Healthcare Administration, Public Health, or a related clinical discipline is required; an advanced clinical degree is preferred.
- Multi-state licensure and care management certification are also preferred.
