We are hiring an experienced Registered Nurse with behavioral health experience to serve as a Disease Manager. This fully remote role focuses on empowering patients to manage chronic and behavioral health conditions through education, coaching, and care coordination.

We are hiring an experienced Registered Nurse with behavioral health experience to serve as a Disease Manager. This fully remote role focuses on empowering patients to manage chronic and behavioral health conditions through education, coaching, and care coordination.
Compensation And Benefits
- $41 per hour
- Full remote role with equipment provided
- Predictable weekday hours between the hours of 9 AM–6 PM MDT
- Stable, mission-driven position supporting military families
- Provide disease management services using evidence-based care plans
- Collaboratively assess, plan, implement and evaluate patient needs
- Coordinate care with an interdisciplinary team to reduce complications and prevent hospitalizations
- Deliver behavioral health support alongside chronic condition coaching
- Current unrestricted, multi-state RN license
- Behavioral health clinical experience
- U.S. Citizen able to pass DoD background investigation
- Minimum of three years of clinical experience
- Prior case management, care coordination, or discharge planning experience
- Disease management experience in managed care
- Bachelor’s degree in nursing or related field
- Familiarity with TRICARE, Medicare or Medicaid
