Position: Patient Care Coordinator - Home Infusion (Remote)

Core Function: This is a comprehensive care coordination role that bridges clinical care, administrative operations, and pharmacy partner communication in the home infusion setting.

Key Responsibilities:

  • Patient Care Coordination: Direct patient interaction and organization of care management throughout the infusion therapy process
  • Administrative Coordination: Managing workflow between healthcare providers and pharmacy services
  • Therapy Oversight: Data entry, maintaining accurate records, staffing PRN nurses
  • Coordinate with nurses and other healthcare professionals: Ensuring a smooth transition of care and communication between different team members.


Ideal Candidate Profile:

  • 1-3 years' experience preferably in infusion/oncology setting
  • Strong relationship management and communication skills
  • Previous experience as a staffing coordinator and/or referral coordinator preferred
  • Detail-oriented
  • Proficiency in using computer systems, databases, and relevant software.
  • Ability to manage complex, multi-party workflows
