Role Description
Zócalo Health is at a pivotal moment of rapid growth and expansion. We are seeking a hands-on, strategic operator with a strong growth mindset to lead and scale our clinical operations across multiple regions. This role demands a leader who thrives in dynamic environments, builds and optimizes systems from the ground up, and empowers frontline teams to deliver exceptional outcomes at scale.
An integral member of the Zócalo Health team, you will oversee the day-to-day and strategic operations across our care coordination, care management, behavioral health, and primary care service lines. This leader will be responsible for designing and subsequently ensuring the efficient, high-quality delivery of care in alignment with value-based care goals, regulatory requirements, and our mission to improve outcomes for vulnerable populations.

The Senior Director, Clinical Operations will play a critical role in mentoring, inspiring, and elevating talent across clinical and clinical operations teams. This leader will ensure that every team member is supported in becoming an accountable owner of their function.
This position will initially report to the CEO and will work in close partnership with Zócalo's future Chief Medical Officer.

The Senior Director, Clinical Operations will contribute in the following ways:
- Build, scale, and continuously improve clinical and care management operations across all service lines (ECM, CHW, behavioral health, and primary care), with a focus on operational excellence, rapid replication, and cross-regional consistency.
- Collaborate closely with clinical leaders to define and operationalize care models that are evidence-based, culturally competent, and measurable.
- Collaborate closely with Regional General Managers to solve local issues and identify local patterns, and participate in regional operating committees as a key stakeholder.
- Design and manage workflows that drive efficient post-acquisition patient onboarding, care coordination, clinical documentation, scheduling, and follow-ups.
- Oversee and improve patient flow and team coordination to reduce drop-offs and improve engagement across the care journey.
- Develop and track KPIs across clinical operations to monitor access, timeliness, quality, and patient outcomes in real-time dashboards.
- Ensure compliance with health plan contracts, managed care regulations, and value-based payment requirements.
- Partner with product and engineering teams to define and implement tech-enabled workflows that enhance team productivity, member engagement, and data capture.
- Hire, develop, and manage a high-performing team.
- Own operational readiness and scalability as Zócalo Health grows across new regions and lines of business. Work closely with executive leadership to stand up new markets quickly and with operational rigor.
- Design and implement performance management systems to support and retain front-line clinical teams, including community health workers, case managers, licensed clinicians, and other key team members.
- Collaborate with compliance to navigate multi-state Medicaid regulatory environments to support successful expansion in new markets.
- Lead the development of scalable infrastructure and operational readiness to enter into delegation, risk arrangements and new VBC models.
- Lead initiatives to drive operational standardization while allowing for local adaptability in emerging regions, ensuring our model scales effectively without compromising on quality or community relevance.
- 8+ years of experience in healthcare operations, including at least 4 years in a senior leadership role.
- Proven track record of building and scaling clinical operations in fast-paced, multi-region or multi-site environments—preferably within Medicaid, value-based care, or complex care delivery models.
- Strong operational instincts with a bias for action and a demonstrated ability to lead through ambiguity, set clear priorities, and drive toward measurable results.
- Deep understanding of clinical workflows and the ability to partner closely with cross-functional teams to turn care models into scalable systems.
- Experience directly supporting or managing interdisciplinary care teams, including primary care, behavioral health, care coordination, and community health workers.
- Growth mindset with a passion for continuous improvement, system design, and replicable infrastructure that balances local nuance with scalable standards.
- Hands-on, data-driven mindset with the ability to dive into raw data, build and interpret dashboards, and independently run analyses using tools like Excel (pivot tables, VLOOKUPs, filters) or SQL. This role requires direct engagement with operational data—not just reviewing reports. You should be comfortable using spreadsheets and queries as tools to identify issues, surface patterns, and test solutions yourself.
- Comfort navigating Medicaid regulations, compliance requirements, and payer relationships across multiple states.
- Experience collaborating with product, engineering, and data teams to design and implement tech-enabled workflows that improve efficiency and engagement.
- Deep commitment to health equity, cultural humility, and frontline team development, with a willingness to engage in-person with field teams to reinforce Zócalo's community-first operating model.
