Position Summary

The Home Health Referral & Intake Coordinator / Fax & Orders Specialist is a remote, part-time role responsible for timely processing of referrals, uploading of faxed physician orders and other clinical documentation, and ensuring all materials are accurately entered and attached in the patient’s chart. This position serves as a communication link between intake and clinical teams by promptly forwarding faxed orders to clinical managers.

Key Responsibilities

Referral & Intake Duties

  • Review, enter, and process new home health referrals during designated work hours.
  • Ensure all referral data is accurately entered into the EMR system.
  • Attach all referral documents to the corresponding patient chart upon entry.
  • Communicate with referral sources to obtain missing information or clarify details.

Fax Management & Documentation

  • Monitor digital fax system for incoming documents (e.g., referrals, orders, visit notes).
  • Upload and accurately label documents in the appropriate patient chart in the EMR.
  • Email clinical managers promptly with any new or time-sensitive faxed physician orders.
  • Maintain accurate records of faxed items processed during each shift.

Physician Orders Coordination

  • Check for returned signed physician orders and attach them correctly in the chart.
  • Track outstanding orders and follow up with provider offices as needed.
  • Ensure all orders are routed to the appropriate clinical team for awareness and compliance.

Skills & Competencies

  • High attention to detail and accuracy in data entry and document management.
  • Strong communication skills via email and phone.
  • Excellent time management and ability to work independently in a remote setting.
  • Familiarity with HIPAA compliance and handling of protected health information (PHI).
  • Proficiency in EMR software and Microsoft Outlook.

Qualifications

  • High school diploma or equivalent required; associate degree preferred.
  • At least 1 year of experience in a home health, medical office, or healthcare intake role.
  • Knowledge of home health documentation standards and Medicare/Medicaid preferred.
  • Must have reliable internet, a secure and quiet work environment, and strong technical skills.

Reports To: Clinical Manager or Director of Nursing

Location: Remote

Work Schedule: Monday – Thursday, 8:00 AM – 12pm (Remote, Part-Time)
