Job Description

Position Overview:

The Diabetes Regional Project Management Lead will be responsible for leading the planning and implementation of initiatives and tactics established as part of their workplan required to achieve established expected deliverables of this role.

The successful candidate will have outstanding written and oral communication skills, be an active listener, and have an ability to engage and manage various stakeholders involved in the planning, care delivery, and management of diabetes throughout the Province. The successful candidate will ideally have a desire to effect positive change in healthcare. The Project Manager must work collaboratively within a team of Leaders within Diabetes Education Programs (DEPs) and within Ontario Health(OH).

In this role you will engage in ethical conduct and strive to understand and appreciate the diversity of patient populations and workforces.

The candidate must be able to travel to various locations throughout the Province and/or be able to coordinate meetings virtually. Primary place of work will be within the successful candidate’s home office

Responsibilities

Reporting to the Director of Nephrology & Chronic Disease, the successful candidate will work closely with DEP team and DEP and OH Leaders to achieve the following deliverables:

  • Conduct a comprehensive capacity and needs assessment of DEPs to identify service gaps and ensure equitable access for high-need populations across the province.
  • Identify and evaluate models for regional coordination, including centralized intake processes and integration with other supports such as Community Chronic Disease Coordinators (CCDCs).
  • Collaborate with Ontario Health and other stakeholders to inform the development of regional models for diabetes care delivery.
  • Lead and manage project timelines, deliverables, and stakeholder engagement activities.
  • Prepare reports, presentations, and recommendations to inform decision-making at the regional and provincial levels.

Requirements

  • University Degree or equivalent in Health Sciences or related field
  • PMP (Project Management Professional) designation (preferred)
  • 3-5 years of project management experience, preferably in a healthcare/clinical environment
  • Excellent interpersonal skills with demonstrated ability to be consultative, ability to establish and maintain effective relationships
  • Ability to adapt to shifting priorities, demands and timelines through analytical and problem-solving capabilities
  • Ability to work independently and to exercise sound judgment
  • Exceptional time management skills, must be able to work and perform duties with little or no supervision and be able to work under pressure to meet deadlines
  • Sound project management skills (including budgeting and cost control, risk management, scheduling and reporting)
  • Sufficient understanding of Diabetes as a Chronic Disease and knowledge of what is entailed in care delivery and management
  • Computer skills with hands-on capabilities in Microsoft: Word, Excel, PowerPoint, Project, and Visio, ability to work with Eclipse is an asset
  • Demonstrates a commitment to customer service when dealing members of the diabetes community, stakeholders, and the public
  • Demonstrates excellent work performance and attendance record
