
Sr. Project Manager - Home Care (Alayacare)

ContractOn behalf of our healthcare client, Affinity Healthcare Solutions is looking for a Project Manager to work on the Transition of multiple instances of a legacy home care information system to a new, unified platform to improve information flow and overall healthcare delivery. Focus areas include Clinical Standardization, Care Planning, Assessments, Scheduling, Billing & Financial Integration, Communication, Business Continuity Planning, and Technical Readiness.
Responsibilities:
• Lead the end-to-end project lifecycle for the implementation of a new, unified home care information system, including initiation, planning, execution, monitoring, and project closure
• Develop and manage detailed project documentation, including workplans, timelines, budgets, risk registers, and resource allocations, ensuring alignment with organizational priorities and care delivery goals
• Collaborate closely with clinical, administrative, and technical teams—including home care providers, IT staff, and operational leaders—to ensure requirements are met and workflows are optimized
• Manage procurement processes, vendor evaluations, and contract negotiations to ensure delivery of a cost-effective and fit-for-purpose solution
• Oversee system design, build, integration, data migration, testing, and rollout across all impacted sites and care teams
• Identify and manage project risks, issues, dependencies, and mitigation strategies to maintain delivery timelines and avoid service disruptions
• Develop and execute change management, training, and stakeholder engagement strategies to support smooth transition and user adoption across clinical and administrative users
• Monitor and report on project performance, including key milestones, financial tracking, and risk reporting, to executive sponsors and steering committees
• Coordinate multiple project workstreams concurrently, including scheduling, care planning, assessments, billing, and technical readiness
• Ensure compliance with healthcare privacy legislation, data security protocols, clinical documentation standards, and operational continuity requirements throughout the project
Qualifications:
• 8+ years of progressive experience in project or program management, with a focus on large-scale health information system implementations in community or home care settings
• 3+ years of experience leading projects involving care planning, scheduling, assessments, or other community-based clinical systems
• Demonstrated success delivering enterprise-level healthcare IT projects with diverse stakeholder groups, including health authorities, operational teams, clinical staff, and technology vendors
• Strong understanding of home care or community health workflows, interdisciplinary team coordination, and patient service delivery models
• Proven experience managing system replacement and modernization initiatives, including legacy system decommissioning, data migration, and user transition planning
• Skilled in leading cross-functional teams across geographically distributed sites, with an ability to manage competing priorities in dynamic healthcare environments
• Excellent communication and stakeholder engagement skills, with experience presenting to executive sponsors and facilitating collaboration across clinical and technical domains
• PMP certification or equivalent project management credentials preferred

• Working knowledge of project management principles and techniques.
