Position: Multi-Specialty Practice Patient Support Specialist

Monogram Health is seeking experienced health care professionals committed to providing excellent patient experiences. The Patient Support Specialist will be charged with ensuring that the patient’s experience with Monogram Health is cohesive and all clinical needs are being met. This individual must be comfortable interfacing with patients via virtual platforms, assisting patients with the setup and use of our telemedicine platform, and developing trusting relationships with patients. He or she will support patients and providers. This individual is a vital asset to the team who is responsible for a panel of patients, along with other members of the care team, and guides patients to meet their preventive, chronic and acute care needs.

Position Requirements

  • The position will be remote
  • Excellent time management skills and ability to demonstrate high attention to detail
  • Electronic health record (EHR) experience; Athena and Salesforce experience preferred
  • Medical Assistant certification preferred
  • Outstanding communication skills with exceptional listening ability
  • Ability to be flexible and available as needed throughout the workday
  • Exceptional interpersonal skills with the ability to develop trusting, collaborative relationships
  • Desire to help patients live a better, healthier life

Roles And Responsibilities

  • Assist in managing the health of your panel of patients and proactively track whether your patients are up to date on preventative measures
  • Assist in the facilitation of referrals, prior authorizations, and lab order fulfillment
  • Proactively follow-up with patients regarding logging vitals (BP, BS, weight), symptom assessments, and scheduling needs
  • Aid patients in the fulfillment, initiation and follow-up for prescriptions
  • Help patients and family members understand their chronic conditions and how to prevent further disease progression
  • Ensure that care provided is collaborative and act to engage specialists as needed
  • Facilitate and confirm referrals and prior authorizations are completed and documented appropriately prior to scheduling visits
  • Complete and track prior authorizations for medications and procedures.
  • Review and ensure completion of necessary diagnostic testing and ensure they are uploaded to the system appropriately (e.g., echocardiograms, EKGs, lab work).
  • Assist clinicians as needed
  • Other duties as assigned
