We are hiring a compassionate and experienced Registered Nurse (RN) Disease Manager with a focus in Behavioral Health to support our client on a federal TRICARE contract. This is a remote, contract-to-hire opportunity for RNs who are dedicated to improving mental health outcomes for TRICARE beneficiaries.

Key Responsibilities

  • Provide telephonic behavioral health disease management and patient support
  • Assess, plan and coordinate care for patients with chronic behavioral health conditions (e.g., depression, PTSD, anxiety)
  • Deliver education and interventions that promote mental wellness and self-management
  • Collaborate with providers and care teams to ensure continuity of care
  • Maintain detailed and timely documentation in electronic systems

Requirements

  • Current, active RN license in California (required)
  • Must reside in the Pacific or Mountain Time Zone
  • Minimum 2 years of recent RN experience in behavioral health/case management
  • Experience working with behavioral health populations (civilian or military)
  • Excellent communication, assessment, and follow-up skills
  • Comfortable working independently in a remote setting
