Position Overview
The Intake Coordinator is responsible for the coordination of new patient referrals, working in collaboration with Account Executives/Liaisons, referral sources, discharge planners, and location staff to ensure a seamless transition to agency services in compliance with Aveanna Healthcare policies, procedures, and federal/state regulations and guidelines.
Schedule Friday - Monday 7:30am-6:00pm EST (Four 10-hour days - benefits eligible)

The Intake Coordinator is responsible for the coordination of new patient referrals, working in collaboration with Account Executives/Liaisons, referral sources, discharge planners, and location staff to ensure a seamless transition to agency services in compliance with Aveanna Healthcare policies, procedures, and federal/state regulations and guidelines.
Schedule Friday - Monday 7:30am-6:00pm EST (Four 10-hour days - benefits eligible)
- This role will support intake and eligibility for all home health locations in Florida and is fully remote, with the requirement to come onsite for training as needed **
- Responds to referral source and office inquires by communicating accurate and complete information
- Coordinates multiple telephone calls from internal/external customers and directs callers or tasks appropriately
- Establishes good working relationships between the location and referral sources to ensure further referrals from that source
- Reviews documentation sent by the referral source for accuracy and completeness
- Documents all forms required to process a referral in its entirety with no missing information
- Enters all referral information in real time into the Electronic Medical Record (EMR) to provide visibility across the team
- Partners with colleagues in the location to staff each case with an appropriate, skilled professional to provide the services requested and ordered
- Photocopy, fax, route, scan, or email information as needed or requested
- Maintains department logs/tracking systems as requested
- Other duties as assigned
- High school diploma or GED
- Proficient in Microsoft Office, including Outlook and TEAMS
- Ability to work after hours, on-call schedule
- Familiarity with the Microsoft operating system, Outlook email is a necessity
- Data entry: 40 words per minute/10,000 keystrokes per hour.
- Ability to communicate accurately and succintly in email.
- Basic knowledge of how the medical industry and insurances function.
- One (1) year previous office work experience preferred
- Healthcare experience a plus
- Bilingual experience
- Familiarity with Microsoft Teams, HomeCare Homebase, Careport portals and EPIC portals a plus
- Multi-tasking
- Fast-paced environment
