We are seeking a committed and skilled Care Coordinator (Certified/Registered Medical Assistant) to join our remote team. In this integral role, you will provide essential patient-centered care coordination services aimed at enhancing health outcomes for chronically ill patients. Key Responsibilities
- Conduct regular phone consultations with patients and healthcare providers to ensure seamless continuity of care.
- Develop, update, and implement personalized care plans in collaboration with multidisciplinary healthcare teams.
- Identify and address care gaps, social determinants of health, and referral resources to optimize patient outcomes.
- Empower patients to effectively manage their chronic conditions, guiding them toward achieving their wellness objectives.
- Maintain productivity by handling approximately 30-50 calls daily while providing high-quality care.
- Ensure patient privacy, confidentiality, and adherence to regulatory standards in all communications.
- Active certification or registration as a Medical Assistant with a minimum of 1 year of relevant experience.
- Proficient in navigating EMR/EHR systems, Microsoft Office, Google Suite, and other digital tools.
- Excellent verbal and written communication skills, demonstrating precise grammar and spelling.
- Strong self-motivation and organizational abilities, with a proven capacity to work independently.
- A valid certification or registration as a Medical Assistant is essential.
- Preferred experience includes roles in call centers, triage, care coordination, or case management.
- Familiarity with the unique needs of geriatric populations is advantageous.
