We are seeking dedicated and compassionate Public Health Nurses (PHNs) to join our dynamic team! As a PHN, you will play a crucial role in working closely with the community to promote health, prevent disease, and address health concerns through direct care, education, and advocacy. Your contributions will help improve population health outcomes by delivering culturally sensitive care, building strong community relationships, and leading essential public health initiatives. We are currently looking for Family Health PHNs. Please find team specific information below.

Family Health

Familiy Health PHNs collaborate with other family health program areas, community and hospital partners to support a smooth transition for all families through pregnancy, birth and early parenting. They co ntribute to program planning, evaluation and the development of program innovations that facilitate integration between family health and other programs, and client centred systems of support. Family Health PHNs l ead and contribute to the maintenance and upkeep of program information, resources, client facing teaching materials, and online content. They c ontribute to activities and outcomes in the public health strategic plan, specifically e nabling active living and healthy eating, promoting mental well-being, and advancing health equity. Family Health PHNs l ead, facilitate and participate in meetings and g roups at the team, work group and community level. Proficiency with Microsoft, booking, and information management systems. May begin their PHN role on any Family Health Division team. This provides a robust orientation experience, facilitates integrated and client centred program delivery, and allows time for program rebuilding.

Here Are The Family Health Positions Currently Available

Healthy Babies Healthy Children

  • Provides in-person visits in the client’s home or community and supports comprehensive assessment, health education, support, referral, case management and service coordination for families experiencing vulnerabilities from the prenatal period, infancy, and through early childhood, or
  • Hybrid role of providing in-person visits in the client’s home or community and provide onsite liaison support to the three hospitals in Peel as part of the Public Health Liaison role. Liaisons collaborate with hospital staff to support screening of families, to identify risk and clients who may benefit from Healthy Babies Healthy Children and Infant Feeding Program support

Infant Feeding Program

  • Provides direct client service including assessment, counselling, education, support and referral to community resources to support success in infant feeding
  • Services are offered in home and within clinic, but may include other community settings

Multichannel Contact Centre

  • Provides direct client service using multiple electronic platforms and formats including telephone, email, etc. to assess, counsel, provide health teaching, and referral to community resources and health providers
  • Liaise with other Family Health program areas to develop and maintain current program information, referral pathways and client facing information and systems

What You Will Do…

  • Apply knowledge regarding concepts of population health status, inequities in health, determinants of health, and strategies for health protection and promotion to guide practice
  • Utilize research and evidence and apply critical appraisal skills to inform policies, programs and practice
  • Provide health information, teaching and counselling to clients
  • Use a holistic approach to assess the health status and functional competence of clients with consideration of the impact of the determinants of health
  • Identify and analyze how clients' culture, attitudes, beliefs and feelings about health influence interventions
  • Establish mutually determined goals with client, including outcome indicators and evaluative measures
  • Facilitate client engagement in identifying and defining health issues using community development and community assessment skills
  • Participate in the planning, developing, implementation and evaluation of community wide and site-specific public health strategies using Evidence Informed Decision Making (EIDM)
  • Advocate for and contribute to the development of healthy public policy
  • Collaborate with partners to identify and address public health issues and service gaps in order to achieve improved health outcomes
  • Develop, foster and facilitate inter-sectoral partnerships and networks using team building, negotiation, conflict management and group facilitation
  • Demonstrate culturally sensitive approaches and strategies with diverse populations
  • Communicate effectively with internal and external stakeholders
  • Promote awareness of the Region of Peel and Public Health services
  • Participate in research, quality assurance initiatives, and mentor-ship opportunities as appropriate
  • Adhere to public health and nursing ethical standards and principles
  • Fulfill legal and professional obligations and responsibilities in accordance with relevant legislation and departmental policies, procedures and medical directives/acts as an ambassador for the Region of Peel, Public Health Services and Public Health Nursing
  • Perform other duties as assigned which support the organizational goals
  • Respond to public health emergencies, significant events and incidents as required by the Health Protection and Promotion Act and Emergency Standards

What The Role Requires

  • A Baccalaureate Degree in Nursing or equivalent recognized by a Canadian University
  • Current license to practice as a Registered Nurse by the College of Nurses of Ontario
  • Current CPR certification, Level C or HCP, is a requirement for Public Health Nurses in Family Health
  • Experience in public health or community health nursing is preferred

Work Mode & Job Location: at 7 120 Hurontario Street, Mississauga, along with the ability to work offsite. Please note most positions require the ability to travel to various sites daily in Peel Region (Mississauga, Brampton & Caledon) for home visits, Peel Region clinics and other community partners.
