Job Description Summary

The primary role for this position is serving as a care coordinator for pregnant and postpartum women with mental health and/or substance use disorder needs. Patients access the program through a statewide Perinatal Psychiatry Access Program and a text message-based screening system. The position entails managing an electronic maternal mental health and substance use disorder screening and follow up program. The care coordinator is responsible for enrolling women in the program by phone, reviewing completed positive screens, contacting those at high risk for mental health and substance use problems and conducting a brief mental health, substance use and social needs assessment and determining the appropriate level of care. This includes risk assessments for suicide, drug overdose and intimate partner violence and based on the information from the risk assessment the care coordinator will complete and facilitate an appropriate safety assessment and plan. Following the brief assessment, the care coordinator will employ motivational interviewing techniques to help facilitate engagement in the care plan. The care coordinator will continue to follow-up with the patient to ensure care is accessed and/or needs are met. The care coordinator communicates the assessment and care plan to a multidisciplinary team of providers via EHR or verbally. Additionally, the care coordinator may assist with implementation of this program at local and remote practices within SC

FLSA: Salaried



Work Schedule: M-F, 8:30AM - 5:00PM; some remote work possible



Job Duties:



  • 35% - Facilitating patient engagement in mental health, substance use and referrals for social determinants of health social through motivational interviewing techniques and continued follow-up. Will also be responsible for scheduling patients for appointments.

  • 35% - Conduct brief mental health, substance use and social determinants of health assessments in line with a social workers scope of practice. Synthesize assessments to create a care plan with appropriate level of care including mental health, substance use and referrals for social determinants of health and/or social services. The care coordinator is responsible for daily review of completed positive screens, contacting those at high risk for mental health and substance use problems and conducting a brief mental health, substance use and social needs assessment and determining the appropriate level of care. This includes risk assessments for suicide, drug overdose and intimate partner violence and based on the information from the risk assessment the care coordinator will complete and facilitate an appropriate safety assessment and plan.

  • 30% - Implementing a mental health and substance use screening and treatment program in obstetric and pediatric practices. Serving as a liaison to obstetric and pediatric faculty and staff at on-site and remote practices to address workflow, communication and patient care.



Additional Job Description


Minimum Requirements: A master's degree in a social work or social welfare program accredited by the Council on Social Work Education or from a social work or social welfare program offered by a regionally accredited institution, plus experience as a professional social worker.
